DOOR BOTTOM SPECIFICATIONS
Look at the hinge side of the door to see the width of the door stop.  Usually there are three widths as indicated by the diagrams.

THRESHOLD STYLE
Indicate if threshold style is smooth or grooved and the width of threshold.

Simply take the measurements required, affix a sticker to an inside edge of the door, and put the information in the spaces provided on this form
